Select this appointment type if you've never been seen as a patient by Blooming Women's Wellness
You will meet with Stephanie for your initial appointment where your health history, lifestyle, and health goals will be discussed in depth. Preliminary treatment plan will also be discussed at this visit.
We will then follow up 1 week later for a 15 minute visit to review your finalized treatment plan. You will receive a written copy of your treatment plan prior to your appointment. This follow up visit is at no additional cost.
Select this appointment type for all other appointments after your New Patient Appointment
At follow up appointments (typically every 1 to 3 months), you will check in with Stephanie and provide feedback on your current treatment plan- what is working? What is not working? What are your current struggles and successes? Where do you need more support? After providing feedback, you will collaborate with Stephanie to update your treatment plan as needed
